Elevated – Grace Healthcare Ltd is based in Mansfield and we provide community based health and social care services across Nottinghamshire. We aim to offer support to a variety of clients including older people, those with a disability and people who need assistance to continue living in their own homes and community.

We at Elevated – Grace Healthcare Ltd, we believe in high quality and exceptional customer service and offers a wealth of personal and functional care and associated domestic services to meet the needs of dependent clients (“Service Users”).

Funding Your Care

Direct Payments

If you or a family member receives care and support through the Council, you will be entitled to a direct payment if you wish. The money is paid into your account directly from the Council to spend on your care and support. Ask your local Authority Social Services department for more information on obtaining Direct payment.

Private Funding

If you or a family member is paying for your own care and support, it is important you select a care provider that can address your wants, needs and choices, to ensure the support received is a person-centred and focused on the chosen desired outcomes.
